Pharmacist Survey - "Vaccination Rates among Community Pharmacists"
You are invited to participate in a survey, entitled "Vaccination Rates among Pharmacists." The study is being conducted by Andy Ruiz, PharmD., and Christopher Frei, Pharm.D., M.Sc. of The University of Texas at Austin College of Pharmacy, 1 University Station A1900, Austin, TX, 78712, 210-567-8371, freic@uthscsa.edu.

Purpose
The purpose of this study is to quantify vaccination rates among pharmacists and determine perceived barriers to vaccination. Your participation in the survey will contribute to a better understanding of vaccination rates among pharmacists. We estimate that it will take about 15 minutes of your time to complete the questionnaire. You are free to contact the investigator at the above address and phone number to discuss the survey.

Risks to participants are considered minimal. There will be no costs for participating, nor will you benefit from participating. The survey is anonymous and does not collect any personal, confidential, or sensitive information. A limited number of research team members will have access to the data during data collection.

Your participation in this survey is voluntary. You may decline to answer any question and you have the right to withdraw from participation at any time without penalty. If you wish to withdraw from the study or have any questions, contact the investigator listed above.
